How to Inject GLP-1 Medications: Step-by-Step Guide with Tips

If you've never self-injected medication before, the idea can feel intimidating. The reality: GLP-1 injections use a tiny needle (typically 4-6mm, thinner than a standard blood draw needle), are subcutaneous (just under the skin, not into muscle or vein), and most patients describe the sensation as a mild pinch that's over in seconds.

Before your first injection

Your GLP-1 medication will arrive as either a pre-filled pen (brand names like Wegovy, Ozempic, Zepbound) or a vial with separate syringes (compounded semaglutide). The injection technique is similar for both, with minor differences in preparation.

Medication storage

  • Unopened: Refrigerate at 36-46°F (2-8°C). Do not freeze. Do not expose to direct sunlight
  • In use (pre-filled pens): Can be kept at room temperature (up to 86°F / 30°C) for up to 28 days after first use. Note the date you start each pen
  • Compounded vials: Follow your pharmacy's specific storage instructions — some require refrigeration throughout use
  • Travel tip: Use an insulated travel case with a cool pack for transport. Never put medication in checked luggage (cargo hold temperatures can freeze it)

Step-by-step injection guide

Step 1: Choose your injection site

GLP-1 medications are injected subcutaneously (into the fat layer under the skin) in one of three areas:

  • Abdomen: Most common. Inject at least 2 inches from the belly button. Avoid the waistband area
  • Front of thigh: Upper outer area. Good alternative if abdominal injection causes discomfort
  • Back of upper arm: Only if someone else is administering. Difficult to self-inject here with proper technique

Rotate sites: Don't inject in the exact same spot each week. Rotate within the chosen area (e.g., left side of abdomen one week, right side the next) to prevent lipodystrophy (fatty tissue changes).

Step 2: Prepare

  1. Wash your hands thoroughly with soap and water
  2. Remove medication from the refrigerator 15-30 minutes before injection (room temperature medication is more comfortable and flows better)
  3. Check the liquid: it should be clear and colorless. Do not use if cloudy, discolored, or contains particles
  4. Clean the injection site with an alcohol swab. Let it air dry completely (injecting through wet alcohol stings)

Step 3: Prepare the dose

Pre-filled pen: Attach a new pen needle, dial your prescribed dose. Some pens require priming (dispensing a small amount to clear air) before first use — check your pen's instructions.

Vial and syringe: Draw up your prescribed dose using the syringe provided by your pharmacy. Pull back the plunger to the correct marking, insert the needle into the vial (inverted), and withdraw the medication. Tap out air bubbles and push the plunger slightly to expel them.

Step 4: Inject

  1. Pinch a fold of skin at the injection site (about 2 inches of skin between your thumb and forefinger)
  2. Insert the needle at a 90-degree angle in one smooth motion. Don't hesitate — a quick, confident insertion is less painful than a slow one
  3. Release the skin fold
  4. Press the pen button or push the syringe plunger slowly and steadily
  5. Hold for 5-10 seconds after the dose is fully delivered (this ensures all medication is deposited under the skin)
  6. Withdraw the needle straight out

Step 5: After injection

  • Do not rub the injection site — this can cause bruising and may affect medication absorption
  • A small drop of blood or medication at the injection site is normal
  • If using a pen, remove and dispose of the needle immediately (never store a pen with the needle attached — it can cause air entry and dose inaccuracy)
  • Dispose of needles in a proper sharps container (available at any pharmacy)

Common troubleshooting

"It hurts"

Some tips to minimize discomfort: Use room-temperature medication (cold medication stings), inject slowly, make sure alcohol has fully dried, try a different injection area, and ensure you're using a fresh needle every time (reused needles become dull and hurt more).

"I see a bruise"

Small bruises at the injection site are common and harmless. They occur when the needle nicks a small blood vessel. To minimize: don't rub the site after injection, rotate injection sites, and apply gentle pressure with a cotton ball if you see blood.

"I missed my injection day"

For weekly GLP-1s (semaglutide, tirzepatide): if you miss your injection day, take it within 5 days of the missed dose, then resume your regular schedule. "Air bubbles in my syringe"

Small air bubbles in a subcutaneous injection are not dangerous (unlike IV injections). However, large air bubbles displace medication and reduce your dose. To remove: hold the syringe needle-up, tap the barrel to move bubbles to the top, and push the plunger slightly to expel them.
