How to reconstitute your peptide medication

Complete guide to mixing bacteriostatic water (BAC water) with lyophilized (freeze-dried) peptide powder. Follow these steps carefully to ensure your medication is safe, effective, and properly concentrated.

Before You Start

What is reconstitution?

Reconstitution is the process of dissolving freeze-dried (lyophilized) peptide powder with bacteriostatic water to create an injectable solution. Your medication ships as a dry powder because it's more stable and has a longer shelf life in this form. Once mixed, the solution is ready for dosing.

  • You only reconstitute once per vial - after mixing, draw multiple doses from the same vial
  • The amount of water you add determines your concentration - this affects how many units you draw per dose
  • Reconstituted medication must be refrigerated and used within 28 days
  • Never shake the vial - shaking denatures (destroys) the peptide

Reconstitution steps

Follow these 8 steps exactly. The entire process takes about 10-15 minutes, including dissolving time.

Wash your hands

Wash thoroughly with soap and warm water for at least 20 seconds. Dry with a clean paper towel (not a hand towel - those harbor bacteria). Set up your workspace on a clean, flat surface.

Check your vials

Inspect the peptide vial - you should see a white or off-white cake or powder at the bottom. If the powder looks discolored (yellow, brown) or the vial is cracked, do not use it. Check the BAC water vial - the liquid should be clear and colorless.

Read the label on your peptide vial. Note the total mg content (e.g., 5 mg, 10 mg, 15 mg). You'll need this to calculate your concentration.

Sterilize both vial tops

Use a fresh alcohol swab to wipe the rubber stopper on the peptide vial. Use a separate swab for the BAC water vial. Wipe firmly in one direction - don't just dab. Let the alcohol air dry completely (10-15 seconds). Do not blow on the stoppers.

Always sterilize vial tops before every needle insertion - even if you just opened the vial. This is your most important contamination prevention step.

Draw bacteriostatic water into the syringe

With a new insulin syringe, push the needle through the center of the BAC water vial's rubber stopper. Invert the vial (flip it upside down) so the liquid covers the needle tip. Slowly pull the plunger to draw the amount of water specified on your dosing card.

How much water to add: Check the reference table below. Common amounts are 1 mL (100 units) or 2 mL (200 units). More water = lower concentration = more units per dose. Less water = higher concentration = fewer units per dose.

Remove air bubbles: hold the syringe needle-up, flick the barrel gently, then push the plunger until a tiny droplet appears at the needle tip. Air bubbles aren't dangerous for subcutaneous injections, but they affect your measured volume.

Add water to the peptide vial - slowly

Insert the needle into the peptide vial through the stopper. Aim the needle at the inside wall of the vial, NOT directly at the powder. Slowly depress the plunger to let the water trickle down the glass wall. The water should drizzle onto the powder, not blast into it.

Never squirt water directly onto the powder. The force can damage the peptide's molecular structure (denaturation). Think "drizzle down the wall," not "spray at the powder."

Let it dissolve - do NOT shake

Remove the syringe. Set the vial on your workspace and wait. The powder will begin dissolving on its own within 2-5 minutes. Never shake the vial. If needed, you can gently:

  • Roll the vial between your palms (not vigorously)
  • Tilt it slowly side to side
  • Swirl gently in small circles

Most peptides dissolve completely in 5-10 minutes. The solution should be completely clear and colorless - like water.

If the solution is cloudy, contains visible particles, or has foam that won't dissipate, do not inject it. Contact your care team for a replacement vial.

Label the vial

Write the following on the vial or a piece of tape:

  • Date reconstituted (e.g., "Mixed 7/15/2026")
  • Concentration (e.g., "5 mg / 2 mL = 2.5 mg/mL")
  • Expiration (28 days from mixing date)

Refrigerate immediately

Place the reconstituted vial upright in your refrigerator at 36-46°F (2-8°C). The center shelf is ideal - avoid the door (temperature fluctuates) and freezer. The vial is now ready for dosing.

Set a phone reminder for 28 days from today. Reconstituted medication must be discarded after 28 days, even if solution remains in the vial.

Troubleshooting

Common mistakes & how to avoid them

I shook the vial - is my medication ruined?
A brief, light shake is unlikely to cause significant damage. Vigorous or prolonged shaking can denature the peptide. Inspect the solution: if it's clear with no persistent foam or particles, it's likely fine. If you see lasting foam, cloudiness, or floating particles, contact your care team for a replacement.
I squirted the water directly onto the powder
If the water was added with moderate force, the peptide may have sustained some damage, but not necessarily complete denaturation. Let the vial dissolve normally. If the solution is clear and particle-free, it's probably fine. For future vials, aim the water at the glass wall, not the powder cake.
The powder won't dissolve completely
Give it more time - up to 30 minutes. Gently roll the vial between your palms periodically. Do not shake or add more water. If stubborn particles remain after 30 minutes of gentle rolling, contact your care team. Do not inject a solution with visible undissolved particles.
I used regular sterile water instead of BAC water
Regular sterile water lacks the benzyl alcohol preservative, so bacteria can grow in the vial with repeated needle entries. If you used sterile water, treat the vial as single-use: draw your dose, then discard the remaining solution. For multi-dose use, you must use bacteriostatic water.
I added the wrong amount of water
Do not try to remove or add water after reconstitution. Instead, recalculate your concentration based on the actual amount added. Example: if you added 3 mL to a 5 mg vial instead of 2 mL, your concentration is 5/3 = 1.67 mg/mL instead of 2.5 mg/mL. Adjust the units you draw accordingly. Contact your care team if you need help recalculating.
There are bubbles in the vial after mixing
Small bubbles are completely normal and harmless. They'll dissipate on their own in the refrigerator within a few hours. Persistent foam (which doesn't go away) is different from bubbles - foam may indicate the peptide was damaged. If foam persists for more than an hour, contact your care team.
